Abstract

High-quality nursing care documentation supports interprofessional communication, accountability, and patient safety, yet its completeness in hospitals remains suboptimal, and head nurse supervision is thought to help maintain it. This study aimed to describe head nurse supervision and the quality of nursing care documentation, and to examine the relationship between them among staff nurses in the Inpatient Installation of RSUD Arjawinangun, Cirebon. This quantitative, descriptive correlational study used a cross-sectional design and involved 57 staff nurses selected through total sampling. Supervision was measured with a questionnaire and documentation quality was assessed through document observation, then the data were analyzed using the Chi-Square test. Supervision was rated good by 36.8% of respondents, fair by 31.6%, and poor by 31.6%, while 68.4% of documentation was of good quality. A significant relationship was found between supervision and documentation quality (p = 0.020; Cramér’s V = 0.370). The proportion of good-quality documentation decreased from 85.7% under good supervision to 72.2% under fair supervision and 44.4% under poor supervision. These findings are consistent with most previous studies, although the relationship is not absolute because documentation is also influenced by other factors. It is concluded that head nurse supervision is related to the quality of nursing care documentation. Hospitals are advised to strengthen structured and consistent supervision, while future studies should control other factors using larger samples.
