Hypertension requires long-term medication adherence, yet non-adherence remains a problem at Kobakma Community Health Center. This study explored three concepts: patients’ understanding of hypertension, treatment barriers, and medication adherence. A descriptive exploratory qualitative design was conducted on 1–10 April 2026. Twelve informants were selected purposively, consisting of eight hypertension patients, two health workers, and two family members. Data were collected through in-depth interviews, observation, and documentation and analyzed using the Miles and Huberman interactive model. Findings showed that understanding was often symptom-based, while barriers included forgetting medication, work activities, and treatment fatigue. These conditions shaped adherence behavior and indicate the need for continuous education and practical adherence support
